From the moment of admission, caring professionals will begin to assess each resident’s unique needs. This includes Nursing, Dietary, Activities, Social Services and Rehabilitation departments. The team at Champaign Urbana Regional Rehab Center™ meets as a group and begins to formulate a plan of care which may include assistance from other healthcare professionals, clergy, support organizations or family members.

Family members are invited to attend personal care plan conferences to discuss, change and modify, when appropriate, the prescribed plan of care for each individual resident.

Our wellness philosophy encourages residents to live a full and balanced lifestyle. A complete calendar of activities and social programs are offered for all resident lifestyles. Highly qualified therapists and assistants inspire our residents to reach for and attain new levels of independence. Champaign Urbana Regional Rehab Center™ is licensed by the state of Illinois and participates in the Medicare and Medicaid Programs as well as other third-party insurance plans, including the Veterans Administration…

Mission Statement

The quality team of Champaign Urbana Regional Rehab Center™ strives to provide compassionate and diversified care in a clean and comfortable environment for those who are unable to completely care for themselves. Our variety of services includes rehabilitation, respite, and long-term nursing care services ranging from minimal to total support.

Our Goals

FLEXIBILITY --- To provide residents of Champaign Urbana Regional Rehab Center™ with a variety of alternatives that may be needed throughout the course of their lives, enabling them to return to and/or maintain their maximum physical, social, mental, and spiritual capacities if medically possible.

HOME-LIKE ATMOSPHERE --- We accomplish this goal by adopting the Pioneer Concept of Care. Reversing the hierarchy of care by placing the resident at the top of the organizational chart, we create a care-taking philosophy as opposed to a traditional medical model.

REHABILITATION --- Provide an individual with the means to regain the necessary skills for them to return to their home or to be as independent as possible while remaining a resident of our “home”.

POSITIVE STAFF RELATIONS --- Our residents come first, then our employees and our belief is that success will follow.
